The first set is good for all around, the second set works well with the Darkness or OuroborosSerpentblade, and the last one I just like to use against something like Gravios since you can pretty much stay under it's belly and tank... especially if it does the sleeping gas you can just guard it + knock allies out of their sleep if they managed to get caught in it. I searched up a video real quick on someone using a Lance, and it looks like they might have a mix of Guard+ and Evade+ from their mix of armor, but I think if you follow this strategy you should do fine!

Just don't get too greedy with attacks, make use of the sidestep and back-hop to avoid it's jumps, and thrust away in it's face. xD

As a rule of thumb, attacks that are easy to block are usually terrible to naturally dodge and vice versa. Keep that in mind + some practice = you'll never need evade+ and guard+ anymore.
Constitution+ is nice to have (decrease stamina consumption of block and hop - VERY convenient) but is also not necessary.
Sharpness+1 and/or Sharp Sword is a personally always something you should aim for.
I'm also a big fan of Hunger Negate but that's just me.
Don't use the following:
- Auto-Guard. It'll put you into more trouble than it's worth, and usually causes unintentional stamina rape... Unless of course you use mega juice.
- Evade Inc. Some say it's good for chasing monsters down, but that's what lance charge is for.
